Sometimes Docs will divvy up the procedure into that which is specifically removal of breast tissue vs. liposuction / sculpting of the area.

Also, there is a risk of having prolapsed nipple if too much tissue it taken out, so some Docs leave some tissue in place to prevent that from happening. (B/c of this, some guys will diet down as much as possible to leave only breast tissue in place so the surgeon has a black n' white target for tissue excision.)
